Withdrawal Request Form

submit this form to withdraw from recurring swim lessons

Recurring lessons require a minimum commitment of 8 lessons. You can remain on the schedule as long as you’d like, but you must notify us of your intent to withdraw via this form by the 5th of the current month to end lessons by the last day of the month. Requests made after the 5th will be processed for withdrawal after the following month.
